*All procedures by appointment, the pre-approval should be done one (1) day before the appointment schedule.

Online Approval of LOA process for Outpatient

* Plan ahead and make sure to send your email with all the details at least 2 days before your scheduled procedure.

* Stay accessible in case our HMO personnel need to contact you during the process for additional details.

	The information & admitting staff will instruct the patient if he/she will be using insurance for the admission and proceed to HMO office.
	The patient/relative will be asked to proceed at HMO office for verification of card limit.
	HMO staff will ask for the requirements (doctor’s order, admission chart, HMO data sheet) and some questions.
	HMO staff will call the respective insurance and inform the liaison officer regarding the pre-approval.
	Once approved, the patient will be instructed to go back to the Information Office.

	Proceed to HMO office
	The patient should bring the doctor’s request/referral, HMO card, 1 valid ID
	The HMO staff will call the insurance for approval of requested test/procedure
	Once approved, the patient will proceed to the designated department for his/her procedure or service

	Email the following details to us: Subject Line: “Patients Name - FOR OPD APPROVAL” Doctors’ Request: Inclusion of diagnosis and exact procedure is a must. HMO Card Details: If not available, please provide the card number, ID Number or certificate of coverage. Valid ID: Must include your birthdate
	Indicate in your email the schedule of your procedure (date & time) , contact number, company name , history and chief complaint.
	Wait for our HMO Department to reach out to you during the processing of your request.
